If we do not have documentation of the required MCV4 and TDAP vaccines for your child, a consent form will be attached to his/her 3rd quarter report card for the Health Department to administer the needed vaccines at school on Thursday, May 10. The vaccinations are free and current 6th graders must have them before the first day of 7th grade. Please be sure to return the consent form to the Health Room by Monday, April 30.
